We are seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join Thrive Mental Health Services for a full-time remote position. This role supports a remote work setting that allows for flexibility while ensuring that our staff remains connected with their peers and clients. Our virtual infrastructure utilizes advanced health information technology, enabling seamless documentation, teletherapy sessions, and collaboration with other healthcare professionals. As part of a broad team dedicated to quality improvement, you will contribute to initiatives that enhance patient outcomes and service delivery.

• Conduct thorough age- and culturally-sensitive biopsychosocial assessments for a diverse patient population, evaluating mental health, substance abuse, and medical needs.
• Provide direct behavioral health services, including individual and group counseling, crisis intervention, and comprehensive treatment planning tailored to each client's unique circumstances.
• Document all patient interactions electronically, including evaluations, progress notes, and treatment documentation, ensuring compliance with health regulations and best practices.
• Act as a liaison between clients and community resources, facilitating referrals to local agencies and support programs, including medication assistance and rehabilitation services.
• Engage with Quality Improvement teams to implement data collection processes and measures to enhance service delivery and track patient outcomes.

• Must possess a valid Licensed Clinical Social Worker (LCSW) license in the state of New York.
• A Master's degree in Social Work is required, along with a minimum of three (3) years of post-licensure experience providing direct clinical services.
• Proficiency in counseling techniques and crisis intervention strategies, demonstrating the ability to make independent clinical judgments.

• Strong background in group facilitation and experience in planning community support programs.
• Familiarity with electronic health record systems to manage patient records efficiently.
