As a Licensed Professional Counselor, your responsibilities will include:

• Conducting comprehensive patient intakes, focusing on diagnostic assessments and functional evaluations, while formulating treatment goals in collaboration with the primary care team.
• Providing brief, strategic interventions for patients requiring mental health services.
• Advising primary care providers on the appropriate management of patients in the primary care setting versus referrals to specialty mental health facilities.
• Initiating follow-up procedures to monitor patient progress and adapt treatment plans as needed.
• Creating relapse prevention strategies and assisting clients in maintaining stable functioning.
• Identifying 'at-risk' patients and implementing plans to prevent the escalation of their mental health conditions.
• Coordinating with other health service providers, ensuring seamless communication and care delivery with proper patient consent.
• Documenting patient progress and diagnostic information consistently in electronic treatment records.
• Participating in case discussions with primary care providers to refine treatment plans, thus enhancing overall patient outcomes.
• Equipping patients with self-management tools and educational resources necessary for active participation in their treatment and recovery.
• Adhering to the Code of Ethical Conduct for Clinical Professionals and maintaining legal and regulatory compliance.

Frontier Behavioral & Mental Health Clinic offers a competitive salary, with a range of USD 87,000 to USD 105,000 annually, commensurate with experience and qualifications. Additional compensation benefits include:
