Moonlight Mental Healthcare Services is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our innovative team. This position is conducted entirely remotely, allowing you to maintain a flexible work-life balance while delivering essential services to clients in a hospice center setting. You will connect with families and interdisciplinary teams through virtual platforms, leveraging technology to provide holistic support tailored to each individual’s circumstances. Your role will primarily involve telehealth sessions and virtual consultations.

• Conduct thorough assessments and evaluations of clients’ emotional and psychosocial needs within the hospice setting.
• Collaborate with interdisciplinary teams to develop and implement comprehensive care plans, ensuring all aspects of the client's well-being are addressed.
• Facilitate ongoing communication and support for families, providing counseling and guidance to help them navigate the emotional challenges associated with hospice care.
• Monitor clients' progress and adjust treatment strategies in collaboration with the care team, maintaining documentation in compliance with regulatory standards.
• Provide education and resources to families to empower them in making informed decisions about care options and supportive resources.
• Develop and lead support groups for families experiencing grief and loss, creating a safe environment for sharing experiences and emotions.
• Stay informed about the latest practices in hospice care and social work to maintain compliance with all Federal and State regulations governing end-of-life services.

This role specifically targets families and individuals facing life-limiting illnesses. You will work closely with infants, toddlers, and their families to address disabilities or developmental delays while navigating the emotional landscape of hospice care. Your expertise will provide critical support, allowing families to process their grief and optimize their time together, ensuring a dignified end-of-life experience.

• A Bachelor’s Degree in Social Work is required; a Master's Degree or Doctorate is preferred.
• Valid LCSW licensure from the Department of Public Health.
• A minimum of one year of experience in a clinical environment, preferably with early intervention services.
• Strong understanding of Natural Learning Environment Practices and how they apply to supportive therapies in hospice care.
• Exceptional organizational, time management, and communication skills.
• Bilingual candidates are highly encouraged to apply, as language skills can enhance client engagement.
• Proficiency in virtual communication tools and electronic health record systems.

Preference will be given to candidates who have strong foundations in early intervention strategies, grief counseling, and experience working with hospice or palliative care settings. Familiarity with evidence-based treatment modalities and a track record of collaborative teamwork will be considered assets in this role.

We offer a competitive salary ranging from USD 75,000 to 80,000 annually, commensurate with experience and qualifications. Additionally, you will benefit from:
