• Conduct comprehensive assessments to evaluate clients’ social, emotional, and environmental challenges within the hospice context.
• Develop and implement individualized service plans that outline specific goals, interventions, and ongoing progress monitoring tailored to each client.
• Provide compassionate counseling and support to individuals and families, utilizing evidence-based practices to address grief and bereavement concerns.
• Collaborate with clients and their families to ensure access to vital community resources and services that enhance their quality of life.
• Engage in crisis intervention, conduct risk assessments, and develop safety plans as necessary to support clients during critical moments.
• Work closely with community agencies, organizations, and healthcare professionals to coordinate comprehensive services and advocate effectively for client needs.
• Participate in community outreach activities to promote agency services and increase awareness of available resources.
• Attend and contribute to case conferences, team meetings, and ongoing staff training initiatives to ensure best practices in care delivery.

This role primarily involves working with individuals and families in a hospice setting, facing the emotional and psychological challenges associated with end-of-life care. • A Master’s degree in Social Work from an accredited institution.
• An active and valid state licensure as a Licensed Clinical Social Worker (LCSW).

• Outstanding communication skills, with an emphasis on cultural competence and the ability to connect authentically with diverse populations.
