• Conduct comprehensive mental health assessments that take into account each individual’s medical conditions affecting their mental well-being.
• Develop and implement integrated treatment plans that address both psychiatric and physical health needs of the patients.
• Facilitate individual and group therapy sessions, personalized to each patient's unique challenges, utilizing evidence-based therapeutic approaches to support their growth.
• Collaborate closely with medical providers to ensure cohesive and effective treatment strategies that lead to improved patient outcomes.
• Provide crisis intervention and support during mental health emergencies, ensuring immediate and effective resolution of acute issues.
• Educate patients about the correlation between mental health and physical wellness, empowering them to actively participate in managing their conditions.
• Maintain precise and confidential documentation in compliance with HIPAA regulations and ethical standards, keeping patient records up-to-date and accurate.
• Actively participate in team meetings, case conferences, and interdisciplinary care planning to ensure well-rounded care delivery.
• Stay informed about the latest research, best practices, and advancements in integrated care to continually improve the therapeutic environment.

This role focuses on a diverse clientele dealing with complex mental health needs and co-occurring medical conditions. Many patients may present with challenges related to autoimmune diseases, chronic pain, or substance use disorders. Your expertise will be instrumental in guiding these individuals toward improved mental health, equipping them with tools and strategies to manage their conditions effectively.

• A Master's or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
• Valid licensure as a Licensed Clinical Social Worker (LCSW), Licensed Professional Clinical Counselor (LPCC), or Licensed Marriage and Family Therapist (LMFT) within the state of California.
• Proven experience working with patients who have co-occurring medical conditions.
• Exceptional interpersonal and communication skills, with the ability to build rapport and trust with diverse populations.
• Strong commitment to maintaining professional boundaries and adhering to ethical guidelines, ensuring patient confidentiality at all times.
• Proficiency with electronic health record systems and computer applications necessary for efficient documentation and communication.

• Experience in a clinical setting, specifically in integrated care models.
• Familiarity with telehealth platforms and virtual therapy techniques.
• Background in trauma-informed care and evidence-based therapeutic modalities.
