• Conduct comprehensive psychosocial assessments for patients upon intake, identifying their emotional, social, and environmental needs.
• Collaborate with the multidisciplinary team to develop personalized treatment plans, presenting recommendations based on individual assessments to ensure tailored care for each patient.
• Participate in regular clinical case conferences, contributing insights and observations to improve patient outcomes and optimize care strategies.
• Design and facilitate educational lectures on chemical dependency, promoting awareness and understanding within the community and among patients.
• Provide individual and group counseling sessions focusing on chemical dependency, tailoring interventions to the unique needs of clients.
• Deliver adolescent and family therapy services as needed, ensuring a holistic approach to family dynamics and support during challenging times.
• Coordinate case management services, directing families towards valuable resources and referrals that will aid their journey in hospice care.

The individuals and families you will work with are often facing significant health challenges, including life-limiting illnesses. Our hospice center serves patients in Pendleton, Oregon, and surrounding areas, ensuring that they receive the emotional and psychological support necessary during end-of-life care. You will engage with diverse demographics, making cultural competence and empathy key attributes for success in this role.

• A Master’s Degree in Social Work (MSW) or a related field is required.
• Licensure as a Licensed Clinical Social Worker (LCSW) in the state of Oregon is mandatory.
• A minimum of two years of documented post-graduate experience in supervised addiction therapy is required, with special consideration given to those with backgrounds in hospice or palliative care.

While not mandatory, candidates with prior experience working in hospice, palliative care, or with patients dealing with chemical dependency will be prioritized. Familiarity with local healthcare resources and a commitment to addressing substance use disorders are also highly valued.
