JOB DESCRIPTION

Insight Global is assisting a client in finding an empathetic and mission driven fully remote Outreach Specialist to engage eligible members and connect them with healthcare and community-based support services. This role serves as the first point of contact for members, helping build trust, identify needs, provide education, and facilitate a seamless transition to ongoing care teams. The ideal candidate is naturally curious, empathetic, and comfortable having meaningful conversations with individuals facing complex health, behavioral health, and social challenges. Success in this role requires strong communication skills, emotional intelligence, and the ability to thrive in a fast-paced, metrics-driven environment.

Responsibilities:

•Conduct outbound outreach to eligible members via phone and text.

•Build relationships and educate members on available services and resources.

•Complete intake activities, obtain required consents, and document member information.

•Assess member needs and connect them with appropriate care team resources.

•Follow up with recently discharged hospital patients and support care transitions.

•Maintain accurate records and communicate member needs to internal care teams.

•Meet outreach, engagement, and enrollment performance metrics

•80-100 outbound calls per day, member engagement, consent completion, conversion rates, quality scores, and SLA compliance.


REQUIRED SKILLS AND EXPERIENCE

•1+ year of experience in outreach, social services, healthcare, insurance, case management support, or a call center environment.

•Experience handling high-volume outbound calls.

•Strong communication, active listening, and relationship-building skills.

•Comfortable discussing sensitive topics and supporting vulnerable populations.

•Ability to thrive in a fast-paced, metrics-driven environment.

•Flexible, adaptable, and passionate about helping others.

•Stable work history


NICE TO HAVE SKILLS AND EXPERIENCE
