• Conduct thorough clinical assessments for patients transitioning to hospice care, identifying psychological and social needs.
• Develop and implement personalized treatment plans based on evidence-based best practices, adjusting them as necessary to meet evolving patient needs.
• Utilize therapeutic techniques, including solution-focused therapy, to empower patients and their families to cope with grief, anxiety, and other emotional challenges.
• Document patient progress and maintain detailed, timely electronic health records (EHRs) to inform medical and support staff.
• Collaborate with a multidisciplinary team, including physicians, nurses, and wellness coordinators, to ensure holistic patient care.
• Provide psychoeducation and emotional support to families navigating the complexities and stressors of hospice care.
• Stay abreast of current research and advancements in social work and hospice care to continually enhance the support provided.

Your role will focus primarily on patients and families in hospice care, a sensitive area where emotional and psychological needs are paramount. You will work with individuals facing terminal illnesses, their caregivers, and family members, addressing diverse needs that span various demographics and backgrounds. • Bachelor's Degree in Social Work from an accredited institution recognized by the Council on Social Work Education.
• Must possess a valid Licensed Independent Clinical Social Worker (LICSW) license in the state of Washington.
• Proven knowledge and application of solution-based therapeutic interventions tailored to hospice care.
• Exceptional interpersonal and communication skills to facilitate rapport-building with patients and team members.

• A minimum of two years of experience in a clinical setting, preferably within hospice, palliative care, or mental health.
• Familiarity with electronic health record systems and remote therapy platforms.
• Previous experience collaborating within a multidisciplinary team for patient-centered care.
