Key Responsibilities
  • Conduct virtual, in-depth psychosocial assessments to evaluate the strengths and needs of clients facing complex emotional and mental health challenges.
  • Utilize evidence-based therapeutic modalities to provide counseling services to individuals, families, and groups, addressing issues common in hospice care, such as grief and loss.
  • Work collaboratively with physicians, nurses, and other healthcare providers to coordinate comprehensive and integrated care plans that promote overall wellness.
  • Advocate for clients’ rights and improve their access to community resources and additional support services.

Qualifications
  • Master's degree in Social Work from an accredited program, ensuring a strong foundation in clinical practice.

  • Aminimum of two years of post-licensure clinical social work experience is required, with a focus on mental health and end-of-life care.
