• Conduct thorough psychosocial assessments to identify the mental health concerns, resources, and strengths of clients.
• Provide individual, couple, and family therapy utilizing evidence-based modalities such as cognitive-behavioral therapy (CBT), solution-focused therapy, and trauma-informed approaches tailored to the hospice setting.
• Design and execute individualized treatment plans that clearly outline goals and strategies for intervention, ensuring alignment with the unique needs of each client and their families.
• Collaborate actively with healthcare professionals, including psychiatrists, nurses, and chaplains, to enhance the overall care provided to clients and ensure a multi-disciplinary approach to treatment.
• Deliver crisis intervention, risk assessments, and safety planning actions, particularly for clients experiencing acute emotional distress.
• Maintain comprehensive and timely documentation in accordance with ethical and legal standards, ensuring clarity in the therapeutic process and facilitating continuity of care.
• Facilitate group therapy sessions focused on grief support and coping mechanisms, as well as psychoeducation courses to empower families in understanding the hospice process.

Your primary focus will be on clients in hospice care, where you will address a spectrum of emotional responses related to end-of-life issues, including grief, anxiety, and adjustment to terminal diagnoses. In this role, you will work compassionately with not just clients but also their families, providing supportive counseling geared towards navigating complex emotional landscapes. Your work directly influences their emotional and psychological resilience as they face significant life transitions.

• A Master’s degree in Social Work from an accredited institution.
• An active California state license as a Licensed Clinical Social Worker (LCSW) is required.
• A minimum of three years of experience in a mental health or outpatient setting, with a preferred focus on hospice or palliative care.
• Strong proficiency in evidence-based therapeutic modalities, with a particular focus on those applicable to grief and trauma.
• Exceptional interpersonal, communication, and clinical assessment abilities, enabling effective engagement with a diverse range of clients.

• Experience working in hospice, palliative care, or related settings is highly desirable.
• Experience in the development and facilitation of therapeutic groups or support sessions.
• Familiarity with electronic health records (EHR) systems to maintain clinical documentation.
