Position Overview

Health Access Network is seeking a dedicated and compassionate Full-Time Clinical Social Worker (LCSW) to join our dynamic team remotely, with a focus on providing critical support within our hospice care facility. Work Environment

This is a full-time remote position allowing flexibility while enabling you to interact and collaborate closely with both colleagues and clients. Key Responsibilities

  • Provide empathetic counseling and therapeutic support to individuals and families experiencing grief, loss, and other complex life transitions associated with hospice care.
  • Actively collaborate with clients to create personalized service plans and connect them with community resources, ensuring they receive the comprehensive care they need.
  • Deliver crisis intervention, conduct risk assessments, and implement safety plans as necessary, prioritizing the well-being of clients and their families.
  • Engage with community agencies to coordinate services and advocate for clients' needs, fostering a strong network of support.
  • Participate in community outreach initiatives to enhance awareness of the agencyโ€™s services and address prevalent social issues affecting our client populations.
  • Contribute to team meetings, case conferences, and ongoing professional development through staff training and workshops.
Patient Population / Client Focus

Your primary focus will be on clients utilizing hospice services, often facing critical life challenges including terminal illnesses, end-of-life planning, grief and loss, and adjustment to life-altering situations. Through your work, you will play an essential role in bringing comfort and support to families, ensuring they have access to the appropriate resources and emotional guidance during difficult times.

Qualifications
  • A Masterโ€™s Degree in Social Work from an accredited program is required.
  • An active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
  • Strong communication skills, cultural competence, and the ability to build trust and rapport with diverse populations.
  • A solid understanding of trauma-informed care, and social and economic justice issues.
Preferred Experience
  • Experience working in a social service agency, particularly in hospice care or a similar setting, is highly preferred.
  • Familiarity with community resources pertinent to healthcare and social services.
  • Previous involvement in case management, crisis intervention, or direct counseling services.
Compensation and Benefits

The annual salary for this position ranges from USD 60,000 to 100,000, based on experience and qualifications, with the potential for bonuses.
