Discovering Peace Counseling Services, LLC is seeking a dedicated and skilled Licensed Mental Health Counselor (LCSW, LMFT, LPC) to join our team on a full-time basis. This remote position plays a critical role in providing psychological support to individuals and families coping with a variety of emotional and mental health challenges. You will be instrumental in educating patients about their conditions, guiding them through the complexities of mental health care, and advocating for their needs within the broader health care system.

This role is entirely remote, allowing for flexibility in your work setting while maintaining a strong connection with our community health initiatives. You will engage in virtual sessions, collaborating closely with other health care professionals and utilizing state-of-the-art digital tools to monitor and manage patient care effectively.

• Provide psychological support and counseling to patients coping with acute, long-term, or terminal mental health issues.
• Educate patients about their treatment plans and mental health conditions, empowering them to take an active role in their recovery.
• Collaborate with community resources and healthcare providers to coordinate care effectively and ensure smooth transitions for patients.
• Advocate for patient rights and access to necessary services, ensuring that vulnerable populations receive the care they deserve.
• Utilize electronic medical records to document patient interactions, track progress towards treatment goals, and align with the interdisciplinary care team.
• Participate in transition planning conferences with patients and families, ensuring they are informed and supported throughout the care process.
• Monitor and report any suspected abuse or vulnerability in adherence to legal and ethical mandates.
• Engage in proactive outreach to identify patients at risk of readmission and implement strategies to mitigate that risk.
• Support families by providing essential information related to community resources, financial aids, and social services.
• Perform other duties as assigned to enhance patient care and support the organizational mission.

The primary client population includes individuals and families facing mental health challenges stemming from various sources, such as trauma, chronic illness, and socio-economic difficulties. Your sensitivity and understanding in navigating these complex issues will be paramount to facilitating healing and supporting your clients’ journey towards mental wellness.

• A valid independent license in your state (LCSW, LMFT, LPC) from an accredited body.
• A Master’s or Doctorate degree in clinical psychology, counseling, or clinical social work from an accredited institution.
• Proficiency in electronic medical record systems, as well as strong communication skills via email and virtual platforms.
• Ability to manage a substantial caseload efficiently, demonstrating an organized approach to meeting patient needs.

• Prior experience in a similar role within a community health setting, ideally in a remote capacity.
• A proven track record of advocating for and enhancing patient care outcomes.
• A passion for making a positive impact on the future of behavioral health care in the community.
