• Conduct thorough psychosocial assessments to gather comprehensive information about clients' mental health needs, strengths, and support systems.
• Provide individual, family, and group therapy sessions using evidence-based treatment modalities tailored to the unique needs of clients dealing with end-of-life issues.
• Implement therapies such as cognitive-behavioral therapy, solution-focused therapy, and trauma-informed practices to address a variety of mental health concerns.
• Develop and manage personalized treatment plans that align with clients’ goals, fostering resilience and coping strategies during their hospice journey.
• Collaborate closely with a dedicated healthcare team, including psychiatrists and psychologists, to ensure an integrated approach to client care and treatment.
• Provide immediate crisis intervention as needed, performing risk assessments and devising safety plans to support clients in acute distress.
• Document all client interactions and treatment outcomes accurately and in a timely manner, adhering to ethical and legal standards.
• Facilitate psychoeducational workshops and support groups to educate clients and families about coping mechanisms and available resources.

You will primarily support clients in a hospice setting, which involves addressing their mental health needs as they navigate terminal illnesses. This role emphasizes working with individuals and families dealing with grief, loss, and existential concerns, allowing you to make a profound impact in a critical phase of their lives.

• A Master's degree in Social Work from an accredited institution.
• Current and active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Virginia.
• A minimum of two years of relevant experience in a mental health or outpatient setting, preferably within hospice or palliative care.
• Strong knowledge of a variety of evidence-based therapeutic techniques and modalities, with the ability to apply them in sensitive, high-stakes situations.
• Exceptional interpersonal, communication, and assessment abilities, enabling effective engagement with clients and their families.

• Experience working in a hospice or palliative care environment is highly desirable.
• Familiarity with interdisciplinary care models and practices.
• Background in providing grief and bereavement support or counseling.
