Key Responsibilities
  • Conduct in-depth psychosocial assessments to understand the needs of patients across various mental, emotional, and behavioral health issues.
  • Develop and implement individualized treatment plans, continually modifying them based on patient progress and changes in status.
  • Collaborate with patientsโ€™ families and other healthcare providers to ensure holistic treatment approaches, assisting in the coordination of care for seamless transitions.
  • Identify at-risk patients who may face avoidable delays or readmissions, and proactively address their complex needs by leveraging community resources.
  • Organize daily workloads and establish priorities by assessing the needs of new, current, and discharging patients effectively.

  • Participate in multidisciplinary team meetings to contribute to discharge planning and continuity of care strategies.
  • Conduct social work consultations, provide crisis intervention, and offer bereavement support as needed.
  • Act as a patient advocate, enhancing communication between patients, families, and care teams to ensure optimal treatment outcomes.
  • Engage in staff support initiatives post-adverse events, delivering necessary psychological care and debriefing sessions.
Patient Population / Client Focus

In this role, you will serve a diverse range of clients, addressing various mental health challenges, including anxiety disorders, depression, trauma, substance use issues, and other behavioral health concerns. Qualifications
