This position offers an annual salary ranging from USD 75,000 to 100,000, plus a competitive sign-on bonus.

Key Responsibilities
  • Conduct detailed psychosocial assessments by engaging in patient interviews, observation, and a thorough review of medical histories to identify emotional, social, and environmental factors affecting recovery.
  • Provide empathetic counseling to patients and families, specifically focusing on navigating adjustment challenges and emotional health concerns.
  • Work alongside the rehabilitation team to create and implement tailored treatment plans that integrate each patient’s psychosocial requirements.
  • Facilitate access to community resources, such as support groups, vocational rehabilitation services, and housing assistance, to ensure comprehensive care post-rehabilitation.
  • Educate patients and families on available community resources and post-hospice services, empowering them to make informed decisions regarding their care.
