• Conduct detailed psychosocial assessments by engaging in patient interviews, observation, and a thorough review of medical histories to identify emotional, social, and environmental factors affecting recovery.
• Provide empathetic counseling to patients and families, specifically focusing on navigating adjustment challenges and emotional health concerns.
• Work alongside the rehabilitation team to create and implement tailored treatment plans that integrate each patient’s psychosocial requirements.
• Facilitate access to community resources, such as support groups, vocational rehabilitation services, and housing assistance, to ensure comprehensive care post-rehabilitation.
• Educate patients and families on available community resources and post-hospice services, empowering them to make informed decisions regarding their care.
• Take an active role in discharge planning, hosting discussions with patients and family members to facilitate seamless transitions to home environments or other care settings.

This position is focused on supporting individuals in hospice care—patients who are dealing with serious, life-limiting illnesses. You will have the opportunity to work closely with patients experiencing various conditions, including cancer, dementia, and progressive neurological disorders. This role involves not just addressing their clinical and functional needs, but also thoroughly understanding and responding to their emotional and social needs, promoting holistic patient care.

• A Master's degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
• Strong understanding of psychosocial dynamics, including grief, anxiety, and coping mechanisms specifically within a healthcare context.
• Exceptional communication and interpersonal abilities, enabling you to establish rapport and trust with patients and their families.

• Prior experience working within a rehabilitation or medical setting focused on hospice or palliative care.
• Familiarity with community resources and support systems beneficial for patients facing end-of-life challenges.
• Experience employing evidence-based therapeutic interventions and frameworks in a clinical setting.
