• Conduct comprehensive assessments to evaluate client needs and develop personalized treatment plans aligned with their goals and circumstances.

• Maintain meticulous clinical records in accordance with state regulations and organizational policies, ensuring confidentiality at all times.

You will work with diverse adult patients facing various challenges related to hospice care. Our clientele may include individuals experiencing grief, adjustment disorders, depression, and anxiety related to terminal illnesses. We aim to create a safe space for clients and their families, fostering open communication and emotional support during difficult times.

• A Master’s degree in Social Work from an accredited institution is required.
• Possession of a valid LCSW license in Illinois is essential.
• A minimum of 2 years’ experience in a clinical setting, preferably with a focus on mental health and hospice care.
• Proficiency in utilizing solution-based psychotherapy techniques and other evidence-based methodologies.

• The salary range for this position is competitive, with annual compensation between $80,000 and $100,000.
