Step to Freedom Counseling Center is seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our compassionate team. • Conduct thorough psychosocial assessments via interviews, observations, and comprehensive medical record reviews to evaluate emotional, social, and environmental factors affecting your patients' recovery.
• Provide empathetic counseling and unwavering support to clients and their families, focusing on addressing adjustment issues related to end-of-life care.
• Partner with the rehabilitation team to formulate personalized treatment plans that align with the psychosocial needs of patients.
• Guide patients and families in accessing essential community resources such as support groups, vocational rehabilitation services, and housing assistance.
• Offer education and advocacy related to community resources and services accessible post-rehabilitation.
• Engage in strategic discharge planning, facilitating seamless transitions from hospice care to home or other care settings.

As an LCSW at Step to Freedom Counseling Center, you will work primarily with patients in hospice care. This includes individuals facing terminal illnesses and their families, where you will address multifaceted needs encompassing emotional, social, and practical challenges. Your role is crucial in creating a therapeutically beneficial environment, encouraging patients to communicate their desires regarding their care and ensuring emotional support during this sensitive period.

• Master’s degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• A deep understanding of psychosocial issues faced by patients in hospice care, particularly related to grief, loss, and adjustment challenges.
• Outstanding communication and interpersonal skills to foster strong relationships with patients, families, and healthcare providers.

• Prior experience in rehabilitation or medical settings, particularly in hospice or palliative care environments, is preferred.
• A background in utilizing evidence-based therapies and methodologies within a therapeutic setting is advantageous.
