The position is structured as a remote role with flexible hours, allowing you to balance your professional responsibilities with personal commitments. While our hospice center predominantly serves clients within Jacksonville, Florida, the remote setting encourages a dedicated and independent work style conducive to thoughtful and focused client care. You will collaborate closely with a diverse team of healthcare professionals, including physicians, nurses, and other social workers, ensuring an intertwined approach to patient management.

• Conduct thorough psychosocial assessments to evaluate clients' mental health needs and develop individualized treatment plans that align with their goals.
• Utilize evidence-based therapeutic practices such as cognitive-behavioral therapy, solution-focused therapy, and trauma-informed care when facilitating sessions with individuals, families, and groups.
• Engage in regular collaboration with multidisciplinary teams, including medical staff and other therapists, to ensure coordinated and comprehensive care for clients.
• Provide crisis intervention, risk assessments, and safety planning for clients experiencing acute distress or emergencies.
• Maintain accurate and timely documentation in compliance with ethical and legal standards, ensuring a clear communication pathway within the care team.
• Facilitate psychoeducational classes or support groups that promote resilience, coping strategies, and community among clients and their families.
• Offer emotional support and advocacy to clients and families, helping them navigate the complexities of hospice care.

Your work will focus on fostering relationships with clients from various backgrounds who are facing significant life challenges related to chronic illnesses and end-of-life experiences. By providing individualized care and facilitating therapeutic interactions, you will empower clients to express their fears, concerns, and desires during one of life’s most sensitive times. Building trust and rapport with families is equally vital in supporting their emotional journey alongside your clients.

• Master's degree in Social Work from an accredited institution, with a focus on clinical practice.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) in Florida.
• A minimum of two years of direct clinical experience in a mental health or outpatient setting, preferably involving hospice or palliative care.
• Strong theoretical knowledge and practical experience with evidence-based therapeutic modalities and treatment approaches.
• Exceptional interpersonal, communication, and assessment skills to effectively engage with clients and collaborate with healthcare providers.

• Experience in hospice or end-of-life care, including familiarity with the unique needs of clients and families during this transitional phase.
• Advanced training or certification in specialized therapeutic modalities relevant to palliative care and grief counseling.
• Proficiency in utilizing digital health tools and electronic health records (EHR) to maintain client information securely and efficiently.
