Key Responsibilities

  • Utilize evidence-based therapeutic practices such as cognitive-behavioral therapy, solution-focused therapy, and trauma-informed care when facilitating sessions with individuals, families, and groups.
  • Engage in regular collaboration with multidisciplinary teams, including medical staff and other therapists, to ensure coordinated and comprehensive care for clients.
  • Provide crisis intervention, risk assessments, and safety planning for clients experiencing acute distress or emergencies.
  • Maintain accurate and timely documentation in compliance with ethical and legal standards, ensuring a clear communication pathway within the care team.
  • Facilitate psychoeducational classes or support groups that promote resilience, coping strategies, and community among clients and their families.
  • Offer emotional support and advocacy to clients and families, helping them navigate the complexities of hospice care.
Patient Population / Client Focus

Your work will focus on fostering relationships with clients from various backgrounds who are facing significant life challenges related to chronic illnesses and end-of-life experiences. By providing individualized care and facilitating therapeutic interactions, you will empower clients to express their fears, concerns, and desires during one of lifeโ€™s most sensitive times. Building trust and rapport with families is equally vital in supporting their emotional journey alongside your clients.

Qualifications
  • Master's degree in Social Work from an accredited institution, with a focus on clinical practice.
  • Active state licensure as a Licensed Clinical Social Worker (LCSW) in Florida.
  • A minimum of two years of direct clinical experience in a mental health or outpatient setting, preferably involving hospice or palliative care.
  • Strong theoretical knowledge and practical experience with evidence-based therapeutic modalities and treatment approaches.
  • Exceptional interpersonal, communication, and assessment skills to effectively engage with clients and collaborate with healthcare providers.
