This full-time position is primarily remote, allowing flexibility while working from home. You will connect with patients and their families through virtual meetings and case management systems, ensuring continuity of care in a supportive setting. As part of our team, you will have access to the latest telehealth technologies and maintain strong communication with fellow team members, including nurses, counselors, and administrative staff, to coordinate high-quality patient care.

• Conduct thorough assessments of patients’ psychosocial needs, evaluating their mental health, emotional wellbeing, and social circumstances to develop personalized care plans.
• Develop and implement evidence-based therapeutic interventions targeted at minimizing distress, optimizing coping strategies, and enhancing quality of life for patients.
• Maintain meticulous and confidential electronic records, documenting patient interactions, progress notes, and treatment updates in compliance with HIPAA regulations.
• Collaborate closely with a multidisciplinary treatment team, including physicians, nurses, and other social workers, to discuss patient progress, treatment adjustments, and resource availability.
• Stay informed about recent developments in social work practices and hospice care approaches, integrating best practices into your daily work.

In this role, you will work with patients experiencing terminal illnesses such as cancer, heart disease, and neurodegenerative disorders, offering support in coping with grief, loss, and complex emotions. You will also provide guidance to families during difficult transitions, helping them navigate the emotional landscape of hospice care and end-of-life decisions.

• Master's degree in Social Work from an accredited program.
• Valid Licensed Certified Social Worker - Clinical (LCSW-C) license in the state of Maryland.
• Demonstrated expertise in clinical social work, with strong clinical assessment and therapeutic skills.
• Proficient in maintaining confidential records and utilizing electronic health record systems.
• Excellent communication, empathy, and problem-solving abilities.

Experience working in hospice care or palliative settings is strongly preferred. Familiarity with solution-oriented therapy approaches for grief, trauma, anxiety, and depression will enhance your effectiveness in this role. Ability to cultivate rapport with patients and families to provide compassionate care is essential.
