This is a fully remote position, allowing you the flexibility to work from the comfort of your home while engaging with patients and the interdisciplinary team through telehealth platforms. • Conduct thorough clinical assessments and develop individualized treatment plans tailored to the unique needs of hospice patients and their families.
• Deliver therapeutic interventions, such as grief counseling, cognitive-behavioral strategies, and supportive psychotherapy, to assist patients in coping with their end-of-life journey.
• Maintain accurate and detailed clinical records while adhering to HIPAA guidelines and ethical standards, ensuring the highest level of confidentiality and professionalism.
• Collaborate with physicians, nurses, and other healthcare professionals to create a unified approach to patient care, addressing both physical and emotional needs.
• Facilitate regular case consultations and participate in interdisciplinary team meetings to enhance treatment strategies and share knowledge regarding best practices.
• Provide education and resources to patients and their families about coping techniques, community services, and available treatments, empowering them to make informed decisions during challenging times.
• Stay informed on the latest developments in social work and palliative care to continually refine care approaches and improve patient outcomes.

In this role, you will primarily work with patients in hospice care, who are navigating significant life transitions. Your understanding of the emotional and psychological challenges faced by terminally ill patients will be crucial in providing compassionate support to both patients and their families. You will play an essential role in enhancing their quality of life by validating their experiences and facilitating meaningful conversations during this critical time.

• A Master’s degree in Social Work (MSW) from an accredited institution, demonstrating a solid foundation in clinical social work practices.
• A valid and active LCSW license in the state of Georgia, signifying your professional competency in providing therapeutic services.
• A minimum of 3 years of relevant clinical experience in the healthcare field, preferably in hospice or palliative care settings.
• Strong knowledge of evidence-based psychotherapy methods and techniques, including grief support and behavioral therapies.
• Exceptional communication and interpersonal skills, allowing you to build rapport and trust with patients and families.
• Proven problem-solving and critical thinking abilities to navigate complex emotional situations and foster positive outcomes.
• A commitment to teamwork, with respect for the collaborative roles of other healthcare professionals in the hospice care setting.

• Experience working in hospice, palliative care, or end-of-life services, coupled with a deep understanding of the unique challenges faced by this patient group.
• Familiarity with telehealth technologies and remote patient engagement strategies to provide effective care.
• Prior involvement in community outreach or support programs related to grief and loss can be an asset.
