• Conduct thorough assessments of seniors’ emotional, mental, and social needs, utilizing structured interviews and standardized assessment tools.
• Collaborate closely with clients and their families to create personalized care plans that honor individual preferences and goals, facilitating regular check-ins to monitor progress.
• Provide empathetic counseling to both seniors and their families, assisting them in navigating complex life transitions and associated challenges, ensuring ongoing emotional support.
• Advocate for clients’ rights, facilitating access to community resources and services that align with their healthcare and social needs.
• Work in conjunction with a multidisciplinary team of healthcare professionals including nurses, hospice aides, and medical staff to coordinate an integrative care approach that addresses all aspects of client well-being.
• Conduct regular remote visits, utilizing telehealth technology to evaluate client conditions, adjust care plans as necessary, and document changes accordingly.
• Educate clients and families on the hospice services available to them, including support groups and social engagement activities that foster community connections.
• Document all client interactions diligently in the electronic health record (EHR) system, maintaining compliance with healthcare regulations and ethical standards.
• Stay abreast of current trends in social work, hospice care policies, and community resources, integrating this knowledge into practice.

You will be working to support clients in a hospice setting, where the emphasis is on providing relief and comfort, addressing any psychosocial issues that may arise as they transition through this stage of life. • Education: A Master's Degree in Social Work from an accredited institution.
• Licensure: Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• Experience: A minimum of two years of social work experience, preferably within a hospice or geriatric care setting.
• Skills: Proficient in assessment techniques, counseling methodologies, and familiar with EHR systems.
• Interpersonal: Strong empathy, communication, and collaboration skills to build rapport with clients, families, and interdisciplinary teams.

Prior experience in hospice care or familiarization with aging-related issues is highly desirable. Knowledge of community resources, geriatric care protocols, and crisis intervention strategies will enhance your effectiveness in this role.
