• Conduct comprehensive psychosocial assessments to evaluate patients' mental health status, strengths, and needs leveraging tools such as standardized assessment instruments and clinical interviewing techniques.
• Provide evidence-based therapeutic interventions, including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), tailored to meet the unique needs of patients and their families.
• Collaborate with a team of psychiatrists, nurses, and other mental health professionals to ensure coordinated and comprehensive care plans, facilitating team meetings to discuss patient progress.
• Educate patients and their families on diagnoses, treatment options, medications, and coping strategies to help demystify complex health conditions.
• Offer individual, group, and family therapies, utilizing evidence-based modalities that foster resilience and emotional healing.
• Develop and maintain dynamic treatment plans in collaboration with the interdisciplinary team, incorporating patient goals and preferences to enhance engagement and adherence to treatment.
• Guide patients through the intricacies of the mental health care system, advocating for their rights and needs in all settings.
• Facilitate therapeutic groups, psychoeducational sessions, and comprehensive discharge planning meetings to support successful transitions and sustained care continuity.

Your primary focus will be the hospice population, a group that presents with varying degrees of emotional distress due to chronic, terminal illnesses. You will work closely with individuals and their families, providing critical support in navigating end-of-life transitions with dignity and respect. This role demands a deep understanding of bereavement processes, terminal illnesses, and the psychological hurdles faced by both patients and their loved ones.

• A Master's degree in Social Work from an accredited institution.
• Current and active state licensure as a Licensed Clinical Social Worker (LCSW) in Connecticut.
• In-depth knowledge of mental health disorders, treatment modalities, crisis intervention strategies, and ethical considerations in social work practice.
• Demonstrated ability to effectively collaborate within a multidisciplinary team and communicate with empathy and professionalism.

• Experience working in hospice or palliative care settings, particularly with terminally ill patients and their families.
• Familiarity with telehealth modalities and electronic health records (EHR) systems for documentation and patient management.
• Skills in facilitating group therapy and psychoeducational workshops designed for patients facing terminal illnesses.
