• Conduct thorough clinical assessments for hospice patients and develop individualized treatment plans that reflect their unique needs and preferences.
• Utilize evidence-based interventions and psychosocial strategies tailored to each patient’s circumstances, ensuring comprehensive mental health support.
• Maintain detailed and accurate records of patient interactions and progress, adhering to regulatory standards while minimizing administrative burdens.
• Collaborate with a diverse treatment team, including nurses, physicians, and other healthcare providers, to ensure an integrated approach to care.
• Provide ongoing education and emotional support to patients and their families, helping them navigate the complex feelings surrounding terminal illness and loss.
• Stay abreast of the latest research and developments in hospice and palliative care methodologies to promote best practices within your role.

You will be working closely with a diverse patient population, including individuals facing terminal illnesses and their families. Your role is critical in addressing the emotional and psychological challenges faced by these patients during a sensitive time in their lives. You will apply your specialized knowledge to deliver compassionate care, ensuring dignity and emotional well-being throughout the end-of-life journey.

• A Master’s degree in Social Work from an accredited university is required.
• Possession of a valid LCSW license in the state of Oregon, ensuring compliance with local regulations.
• Proficient knowledge of solution-focused psychotherapy techniques and experience in working with families impacted by terminal illness.
• Exceptional communication and interpersonal skills, enabling effective collaboration with patients, families, and multi-disciplinary teams.

The ideal candidate will have at least two years of professional experience in a mental health setting, preferably with a focus on hospice or palliative care. Previous work involving family counseling, grief support, and an understanding of cultural sensitivities related to end-of-life issues will be advantageous.
