• Conduct thorough assessments of patients' emotional, social, and psychological needs and develop individualized treatment plans tailored to each patient's unique circumstances.
• Deliver evidence-based therapeutic interventions, including solution-focused therapy and other psychotherapeutic methods, to help patients cope with anxiety, grief, and depression.
• Maintain accurate and confidential clinical documentation in compliance with HIPAA regulations, ensuring careful tracking of patient progress and treatment outcomes.
• Collaborate effectively with a multidisciplinary team to coordinate care, share insights, and develop comprehensive strategies that support patients and their families through the hospice process.
• Engage in ongoing education and training, remaining up-to-date on the latest research and best practices in mental health and hospice care.
• Provide education and support to families, helping them navigate the emotional landscape of caring for a loved one in hospice.

Your work will primarily focus on providing emotional and psychological support to patients diagnosed with terminal illnesses and their family members. This role is vital in managing the complex emotional challenges that arise in hospice care, including grief, loss, and existential concerns, ensuring that patients feel understood and supported during their final journey.

• A Master’s degree in Social Work from an accredited institution.
• Current and valid LICSW (Licensed Independent Clinical Social Worker) license in the state of Massachusetts.
• At least two years of clinical experience working in mental health settings, preferably within hospice or palliative care.
• Strong knowledge and proficiency in evidence-based psychotherapeutic interventions.
• Exceptional communication and interpersonal skills, with the ability to build rapport with patients, families, and colleagues.
• Analytical thinking and problem-solving capabilities, with a focus on patient-centered approaches.
• Ability to thrive in a collaborative team environment, demonstrating empathy and respect for the diverse backgrounds of all patients.

• Previous experience working in hospice or palliative care settings is highly preferred.
• Familiarity with trauma-informed care models and techniques.
• Experience in providing education and support to families in crisis situations.
• Knowledge of additional languages is a plus, enhancing communication with diverse patient populations.
