• Conduct thorough psychosocial assessments to understand clients’ mental health challenges, strengths, and resources that inform treatment plans.
• Deliver individualized, family, and group therapy by employing evidence-based modalities such as cognitive-behavioral therapy, solution-focused therapy, and trauma-informed practices.
• Develop and execute individualized treatment plans that outline specific therapeutic goals and interventions aligned with clients’ unique circumstances.
• Build collaborative relationships with psychiatrists, psychologists, and other healthcare providers to ensure integrated and holistic care pathways for clients.
• Provide crisis intervention and develop safety plans in acute situations, ensuring clients' immediate needs are met effectively.
• Maintain accurate and timely documentation of therapy sessions, progress notes, and treatment plans in compliance with legal and ethical standards.
• Facilitate group therapy and psychoeducation classes to enhance community support and provide additional coping tools for clients and their families.

In this role, you will primarily work with clients facing serious and terminal health challenges, as well as their families. You will offer psychological support to individuals coping with grief, loss, and existential concerns. The diversity within our patient population means you will engage with individuals from various cultural and socioeconomic backgrounds, requiring a delicate and personalized approach to mental health care.

• A Master’s degree in Social Work from an accredited institution.
• Current and active state licensure as a Licensed Clinical Social Worker (LCSW).
• At least two years of experience providing mental health services, particularly within outpatient or hospice settings.
• Proficiency in evidence-based therapeutic methods and a deep understanding of mental health disorders.
• Exceptional communication and interpersonal skills, enabling the establishment of trusting therapeutic relationships.

While the minimum required experience is two years, we welcome candidates with additional qualifications such as advanced training in trauma-informed care, experience working with grief counseling, or certifications in specialized therapeutic approaches. Familiarity with electronic health records (EHR) and telehealth platforms is strongly preferred, as these systems will be crucial for effective documentation and client interactions in a remote work environment.
