About the Role
As a full-time NP or PA, you’ll deliver comprehensive care to a high-need, high-risk geriatric population. We’re looking for someone with a background in Internal Medicine, Family Medicine, or Geriatrics who can work autonomously while collaborating with our multidisciplinary team. This role combines hands-on patient care with remote support, like prescription refills, lab reviews, and patient communication, giving you a balanced schedule.

Key Responsibilities

  • Conduct thorough physical exams and wellness assessments for seniors
  • Follow up on care plans to ensure the best possible outcomes
  • Propose and manage treatment plans for chronic and acute conditions
  • Maintain accurate patient records and manage appointments effectively
  • Educate patients on preventative care, treatments, and healthy lifestyle choices
  • Engage in both fieldwork and remote care, offering prescription refills, lab reviews, and patient outreach
  • Collaborate with a supportive team dedicated to holistic care

Qualifications

  • Completion of an accredited NP or PA program
  • Experience in geriatrics, internal medicine, or family practice
  • Strong communication, active listening, and analytical skills
  • Compassionate and committed to providing patient-centered care
  • Motivated to serve a geriatric population with complex care needs
  • Ability to work 40 hours a week. 2 days on-site and the other 3 days remote for 2 hours a day
