Key Responsibilities

• Conduct comprehensive mental health assessments tailored to individual needs, taking into account any co-occurring medical conditions that may influence mental wellness.
• Develop, implement, and regularly review integrated treatment plans that address the psychiatric and medical dimensions of our patients’ care.
• Facilitate both individual and group therapy sessions employing evidence-based approaches and techniques that foster personal growth and resilience.
• Work collaboratively with a multidisciplinary team of medical providers, ensuring coordinated care that optimizes treatment outcomes.
• Provide immediate crisis intervention and support to individuals experiencing mental health emergencies, utilizing appropriate de-escalation techniques.
• Educate patients about the intersection of mental health and physical wellness, empowering them with knowledge to manage their conditions effectively.
• Maintain meticulous documentation of patient interactions and treatment plans while adhering to all legal and ethical standards.
• Engage in regular team meetings and case conferences to share insights and collaborate on patient care directives.
• Stay informed about current research, best practices, and advancements in integrated health services.

Patient Population / Client Focus

You will be responsible for delivering care to a diverse population, including individuals facing various complex mental health issues, compounded by medical challenges such as chronic illnesses, substance use, and pain management. Our patients seek help for anxiety, depression, trauma, and other mental health concerns, necessitating a sensitive and customized approach that aligns with their unique contexts and struggles.

Qualifications

• Master’s or doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, Psychology, or a related field from an accredited institution.
• Possession of a current and valid licensure as a Licensed Marriage and Family Therapist (LMFT), Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), Licensed Psychoanalyst, or Psychologist.
• Demonstrated experience working with individuals experiencing co-occurring medical and mental health conditions.
• Strong interpersonal and communication skills to foster therapeutic relationships with patients.
• Ability to adhere to ethical guidelines and maintain professional boundaries while ensuring confidentiality.
• Proficiency with computer applications and electronic health record systems.

Preferred Experience

• Previous experience in a telehealth setting or with remote patient interactions.
• Familiarity with evidence-based therapeutic modalities and practices.
• Knowledge of integrated care models and interdisciplinary team collaboration.

Compensation and Benefits
