Job Title: Case Management Coordinator Location: Seminole County, FL (Remote with Local Travel Required) Important Requirement: Candidates must reside in Seminole County , FL (no exceptions) Pay: $34.13 per hour – Biweekly pay Schedule: Monday – Friday | 8:00 AM – 5:00 PM What You’ll Do Manage a caseload of members, including medically complex populations Conduct comprehensive assessments (telephonic and face-to-face) Develop, implement, and monitor individualized care plans Coordinate care with PCPs, specialists, and community resources Assist with prior authorizations, medication reviews, and condition management Support members through education, coaching, and motivational interviewing Identify and escalate quality of care concerns when appropriate Document and track care management activities in compliance with guidelines Facilitate multidisciplinary collaboration to ensure optimal outcomes Work Environment Approximately 75% local travel within Seminole County (home visits, assisted living facilities, nursing homes) Remote training conducted via Microsoft Teams Required Qualifications Must reside in Seminole County, FL Bachelor’s degree required (Social Work or related field preferred; no nursing candidates) Minimum 1 year of case management experience (required) Bilingual (Spanish/English) *
