Job Overview
The Case Manager I is responsible for various tasks facilitating the client's ability to engage in and remain in medical care. The Case Manager's specific tasks vary depending on the program to which the staff member is assigned. The Case Manager's duties include patient advocacy, applying for public benefits, completing applications for the AIDS Drug Assistance Program (ADAP), determining Ryan White eligibility, providing outreach to clients at risk of or who have fallen out of care, scheduling transportation, and completing monthly reports and special projects. The Case Manager I works as an integral part of the Psychosocial Program staff.
Minimum Requirements
  • A high school diploma is required.
  • A bachelor's degree in Human Services or a related field is preferred.
  • Minimum of 1 year of case management experience or equivalent work experience within a related field of Health and Human Services is required.
  • Must be certified through the Los Angeles County HIV Case Management Certification process upon employment.
  • A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Compensation
$28.84 - $36.05 hourly
