Job description

Qualified candidates must live in PA and be able to travel throughout the entire state same day as needed.

For roles that are 100% remote or hybrid, you must have access to a reliable high-speed internet connection to support daily job responsibilities. A minimum bandwidth of 50 Mbps download and 5 Mbps upload is required. Those fully remote associates residing in states where service is required by contract, law, or regulation will be allowed to submit for reimbursement.

Role Overview

The Manager will focus on departmental integrity, learning, and data management. They will also be the  Community HealthChoices Long-Term Service and Support subject matter expert, ensure compliance with program and contractual requirements, assist with developing and managing policies and procedures, and ensure alignment with State and Federal Requirements. 

Work Arrangement

  • Remote
  • Qualified candidates must live in PA and be able to travel throughout the entire state same day as needed. 
     

Responsibilities

  • Conduct audits related to departmental compliance with LTSS policies
  • Assists in the day-to-day management of the department and participates as an internal SME as directed
  • Development of workflow and desk-level tools for departmental education, training, and ongoing in-services
  • Assist in coordinating provider, staff, and member outreach, education, and training
  • Develop and coordinate departmental communication pieces for both internal and external stakeholders
  • Prepares, coordinates, and verifies data and reports for the LTSS department
  • Assures that outbound and inbound data are QA’d and prepared for submission promptly
  • Maintains and presents key metrics to departmental leadership routinely as directed
  • Directly oversee the transitional services function within the department
  • As needed in new business situations, assume the role of phasing in the State’s Home and community-based Service system, roles, and responsibilities


Education/Experience

  • Bachelor’s Degree required.
  • 3 to 5 years of experience with HCBS Waivers, Habilitation, Institutional Services, and Health Homes.
  • 3 to 5 years of Leadership and Management experience required.

Skills & Abilities

  • Knowledgeable in preparing educational materials, presentations, and overall adult learning methodologies.
  • Knowledgeable in basic data analytics related to aggregating, distinguishing, and displaying data for reporting purposes.
