Key Responsibilities
  • Conduct ongoing patient assessments, tailoring care and interventions to meet individual needs.
  • Maintain effective communication with various stakeholders, including supervisors, direct care staff, family members, and guardians, regarding patient progress and treatment plans.
  • Provide a tentative schedule of appointments, home visits, and observations to ensure comprehensive monitoring of patient progress.
  • Coordinate and conduct home visits to gather data through observations and interviews that contribute to effective care planning.
  • Implement individualized program plans and monitor their effectiveness to achieve optimal patient outcomes.
  • Engage actively in Interdisciplinary Team Meetings to collaborate on patient care strategies.
  • Prepare and complete monthly and quarterly progress notes that document patient advancements and plan revisions.
  • Organize training sessions for staff and team members on program implementation and necessary documentation protocols.
  • Provide follow-up interviews and additional support through home visits to address ongoing patient needs.
  • Act as a liaison with community members and mental health organizations to support and further develop treatment goals.
