In this full-time remote role, you will work from the comfort of your home while having access to a supportive virtual team environment. • Conduct thorough biopsychosocial assessments to determine the unique needs of each patient, allowing you to create tailored treatment plans.
• Provide both short-term and long-term therapeutic interventions designed specifically for a telehealth context, ensuring effectiveness and accessibility.
• Coordinate patient care with various stakeholders, including hospital staff, outpatient providers, and community organizations to facilitate seamless transitions and comprehensive support.
• Assist patients and their families in navigating complex healthcare systems, including understanding insurance options, accessing financial assistance, and connecting with necessary social services.
• Facilitate online support groups and psychoeducational workshops to promote mental health awareness and provide valuable community resources.
• Address essential social determinants of health that influence patient recovery, such as employment, housing stability, and access to healthcare.
• Advocate for patient rights, ensuring culturally competent and patient-centered care is a priority throughout your interactions.
• Maintain accurate, confidential, and compliant documentation in accordance with healthcare regulations and organizational policies.
• Participate actively in interdisciplinary team meetings, case conferences, and professional development activities to enhance personal and team-wide growth.

You will primarily serve clients experiencing a range of mental health conditions, including anxiety, depression, trauma, and more complex behavioral health issues. Your clients may include individuals from diverse backgrounds, and your ability to practice with cultural humility and sensitivity will be crucial in building trust and rapport. • A valid and active California LCSW or LMFT license is required for this position.

• A strong capacity for developing and implementing functional treatment plans that align with best practices in mental health care.
• Must carry personal and professional liability insurance to ensure compliance and protection in practice.

• A minimum of 2 years of clinical experience in mental health counseling is preferred, particularly within a telehealth or integrated care setting.
• Experience working with diverse populations and a strong understanding of the social determinants of health that affect patient outcomes.
