Key Responsibilities
  • Conduct thorough psychosocial assessments for clients and their families at the start of their treatment and periodically thereafter as clinically indicated. Utilize social work theories and techniques to evaluate the social, emotional, and physical needs of clients.
  • Collaborate with a diverse team of healthcare professionals to create, implement, and adjust psychosocial treatment plans for clients, ensuring seamless referrals to specialty and social services, such as entitlement programs.
  • Provide supportive counseling, grief support, and crisis intervention, helping clients and families navigate the psychological challenges of illness and improve their overall resilience.
  • Independently manage a caseload of clients with varying clinical acuity while engaging in proactive case finding to identify individuals who may benefit from psychosocial support.
  • Facilitate psychoeducational sessions for clients and families, guiding them through adaptation processes and enhancing their understanding of diagnoses, treatment, and support systems.
  • Engage in quality assurance measures related to social work services, contributing to the development and review of policies that improve client care outcomes.
  • Make mental health diagnoses in line with treatment plans as necessary, providing clarity and direction in clientsโ€™ therapeutic journeys.
  • Provide clinical supervision to Masterโ€™s level social workers (MSWs) and student interns, fostering their professional growth through mentorship.
  • Prepare and maintain accurate electronic medical records and progress notes according to organizational standards, ensuring compliance with health department policies.
Patient Population / Client Focus

Our clientele ranges from adults facing mental health challenges to geriatric patients dealing with life transitions, and pediatric as well as adolescent clients grappling with developmental and emotional issues. As a licensed counselor, you will address a variety of mental health needs through compassionate and specialized care tailored to each client's unique circumstances.

Qualifications
  • A valid license in California as an LCSW, LMFT, or LPCC is required.
  • A Masterโ€™s degree in social work, counseling, psychology, or a related behavioral science from an accredited institution is essential.
  • Demonstrated knowledge of effective case assessment, interviewing, and counseling techniques is important for success in this role.
