• Conduct comprehensive mental health assessments of patients, which include detailed psychosocial evaluations, diagnostic interviews, and risk assessments to understand their unique circumstances.
• Develop tailored treatment plans collaboratively with patients that incorporate their personal goals, cultural backgrounds, and specific needs.
• Implement evidence-based therapeutic interventions such as cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and solution-focused brief therapy (SFBT) across individual, couple, family, and group settings.
• Provide immediate crisis intervention services and maintain open communication with other healthcare professionals to coordinate patient care during acute distress situations.
• Maintain thorough and accurate clinical documentation, including progress notes, treatment plans, and discharge summaries, in compliance with organizational policies and relevant regulations.
• Facilitate psychoeducation sessions for patients and their families to empower them with the knowledge and skills necessary to manage their mental health effectively.
• Engage in collaborative practice with a multidisciplinary team—including psychiatrists, psychologists, nurses, and case managers—to ensure comprehensive and continuous care for patients.
• Regularly assess treatment outcomes to gauge the effectiveness of interventions, adjusting plans as needed to better meet patient needs.
• Stay current on best practices, emerging therapies, and research in the mental health field, applying new knowledge to enhance clinical practice.
• Uphold ethical standards and maintain confidentiality in all interactions and documentation concerning patient information.

In this role, you will primarily work with hospice patients who are experiencing significant emotional challenges related to their health conditions. You will support individuals facing end-of-life issues, their families, and caregivers, ensuring that all provided services are empathetic and culturally appropriate.

• A Master’s degree in Social Work from an accredited institution.
• An active State license as a Licensed Clinical Social Worker (LCSW) in California.
• Strong ability to function independently while also contributing effectively within a multidisciplinary team environment.
• Exceptional interpersonal and communication skills evidenced by the ability to establish rapport with patients from all walks of life.

• Experience working with hospice or end-of-life care is highly desirable.
• Knowledge of trauma-informed care practices and cultural competence.
• Familiarity with telehealth platforms and electronic health records (EHR) systems.
