As a Licensed Professional Counselor at Thrive Hard, you will:

• Collaborate with primary care providers to identify, treat, and manage mental health and psychosocial issues, contributing to a comprehensive approach to healthcare.
• Conduct thorough patient intakes, focusing on diagnostic evaluations that guide treatment planning and coordination with the primary care team.
• Provide focused and brief interventions tailored to individual client needs, ensuring timely support for those seeking mental health services.
• Evaluate which clients may benefit from transfer to specialty mental health facilities, and facilitate such referrals when appropriate.
• Initiate follow-up communications to assess patient progress and make necessary adjustments to treatment plans to optimize outcomes.
• Develop relapse prevention strategies and assist clients in maintaining stability through actionable plans.
• Monitor and coordinate the delivery of behavioral health services, linking patients to additional support systems with their consent.
• Document progress and any pertinent diagnostic information in electronic treatment charts, ensuring compliance with agency and healthcare regulations.
• Educate and empower clients to adopt healthier lifestyle choices, aiding them in managing their mental health and physical well-being.
• Engage in interdisciplinary communication with primary care providers to enhance care continuity and support patient recovery.
• Adhere to the Code of Ethical Conduct for Clinical Professionals and uphold professional standards of behavior throughout all interactions.

• A Master’s Degree in Clinical Counseling or a closely related field.
• A current LPC (Licensed Professional Counselor) License.

Thrive Hard Behavioral Counseling Center offers a competitive salary range of USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
