• Collaborate closely with an interdisciplinary team to ensure personalized care plans effectively meet patients' needs.
• Provide empathetic support and thorough information to patients and families regarding their transition plans and necessary next steps.
• Maintain an up-to-date knowledge of various government and private payer networks to facilitate smooth transitions of care.
• Coordinate care by actively collaborating with community organizations and healthcare resources to address patient needs.
• Advocate for patient rights and access to protective services, ensuring ethical decision-making is upheld throughout the therapeutic process.
• Document and report any concerns regarding patient vulnerability or suspected abuse in compliance with legal mandates.
• Participate in care system processes aimed at reducing readmissions through effective patient assessments and interventions.
• Organize and take part in transition conferences, fostering open communication with patients and their families about ongoing care strategies.
• Utilize telehealth tools and electronic medical records (EMR) to monitor and document patient progress as well as enhance communication with other care providers.
• Contribute to program evaluations by participating in quality improvement initiatives and assessing service delivery outcomes.
• Other related duties as assigned, tailored to the evolving needs of the clinic.

This role centers around a diverse patient population, including individuals from various backgrounds facing complex mental health challenges. The therapist will work with patients experiencing issues related to chronic illnesses, mental health disorders, family dynamics, and social determinants of health. Focused on holistic healing, the therapist will provide tailored interventions that support not only the individual but also their families as they navigate care together.

• An independent mental health license (LCSW, LMFT, LPC) in the state of residence, signifying professional qualifications for clinical practice.
• A master’s or doctoral degree in clinical psychology, counseling, or clinical social work from an accredited institution (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD).
• Proficiency in electronic medical records and telehealth technologies to enhance patient care and streamline administrative tasks.
• Strong written and verbal communication skills, particularly via email, ensuring effective correspondence with patients and team members.
• A proven ability to manage a substantial caseload while maintaining quality interactions with patients and collaborating with large healthcare teams.

• Experience in a community health setting or similar environments focusing on integrated behavioral health.
• Demonstrated expertise in working with an interdisciplinary team and managing complex patient care scenarios.
• A background in providing counseling for chronic illness, trauma, or other significant mental health issues is highly advantageous.
• Familiarity with evidence-based practices and current trends in mental health care delivery.
