Garden Path Counseling is seeking a compassionate and skilled Licensed Clinical Social Worker (LCSW) to join our dedicated team. This position is a fully remote role, allowing for flexibility while ensuring that you can engage effectively with patients through teletherapy and digital communication platforms. The hospice center team is composed of a multidisciplinary group of professionals, ensuring a comprehensive approach to patient care, blending social work with medical and psychological support. You will have access to a variety of digital tools and platforms that streamline collaboration and documentation, fostering an efficient and supportive work environment.

• Conduct individual and group therapy sessions, utilizing evidence-based practices tailored to patient needs.
• Assess patients’ mental health status and develop personalized treatment plans based on comprehensive evaluations.
• Maintain accurate and timely documentation of patient progress, treatment strategies, and assessments in adherence to clinical standards.
• Engage in case collaboration with medical staff and other professionals to ensure a holistic approach to patient care.
• Manage crisis intervention situations with skill and empathy, providing immediate support and guidance.
• Participate in regular team meetings and clinical supervision to enhance service quality and professional development.

This role primarily serves patients within a hospice setting, focusing on clients facing terminal illnesses and complex emotional responses to their conditions. You will provide critical support to patients and their families, helping them navigate grief, loss, and other challenging mental health issues. A key aspect of this position is to create a safe and non-judgmental space where patients can express their feelings and discuss their fears and hopes openly.

• Active and current Pennsylvania LCSW license required.
• Bachelor’s degree in Social Work or a related field, with a focus on Mental Health and Counseling preferred.
• Minimum of 2 years of direct experience in social work related to mental health, ideally in an outpatient or hospice environment.

• Proven experience in providing outpatient therapy.
• Training in motivational interviewing or other counseling techniques.
• Clinical experience working with children, teenagers, or the elderly.
• Competence in crisis intervention and managing complex emotional situations.
• Experience in addressing gender-related issues within counseling.
