Work Environment

This position is designed to be fully remote, allowing you to provide necessary support while enjoying the flexibility of working from home. As part of a multidisciplinary team at a Hospice Center, you will collaborate with healthcare professionals to offer holistic care to patients, ensuring their social and emotional needs are addressed alongside their physical health.

Key Responsibilities
  • Conduct individual and group therapy sessions utilizing evidence-based practices tailored to the specific needs of clients.
  • Assess patients' mental health concerns, develop comprehensive treatment plans, and monitor progress with meticulous documentation.
  • Facilitate case consultations and maintain effective communication with other healthcare providers to deliver integrated care solutions.
  • Advocate for clients within the healthcare system to address barriers to treatment and support their overall well-being.
  • Ensure all clinical documentation is completed promptly and in accordance with applicable regulations and best practices.
Patient Population / Client Focus

Your role will primarily focus on patients dealing with various mental health issues, including but not limited to anxiety, depression, grief, and adjustment disorders. Given the Hospice Center setting, you will work closely with terminally ill patients and their families, providing palliative emotional care and support during a critical transition period in their lives.

Qualifications
  • A Master's degree in Social Work (MSW) from an accredited institution.
  • An active and current LCSW license in Pennsylvania.
  • A minimum of 2 years of relevant experience in clinical social work or a related field.
  • Strong proficiency in medical documentation and record-keeping adhering to organizational policies.
Preferred Experience
  • Previous experience in outpatient mental health settings, particularly working with diverse populations.
  • Familiarity with crisis intervention techniques and management of complex cases.
  • Knowledge of gender issues and the impacts on mental health treatment preferences and outcomes.
  • Training in motivational interviewing or other advanced therapeutic methodologies is highly regarded.
