Key Responsibilities

• Serve as a vital liaison between families and outpatient providers, facilitating effective communication and care coordination.
• Engage with insurance companies to navigate pre-certification and utilization management processes, ensuring that patients have access to necessary services.
• Participate actively in discharge and aftercare planning with a focus on holistic patient recovery and family involvement.
• Provide therapeutic meal support, modeling and encouraging healthy relationships with food as part of overall wellness.
• Deliver empathetic and affirming care tailored to each client's unique circumstances and intersecting identities, fostering an inclusive and supportive environment.

Patient Population / Client Focus

This role primarily focuses on older adults in a hospice setting. You will work closely with clients and their families, helping them navigate the emotional and psychological challenges associated with end-of-life care. Your expertise in geriatric social work will be crucial in addressing the complex needs of this population.

Qualifications

• Education: A Master's degree in Social Work from an accredited institution.
• Licensure: A valid LCSW license in the state of Texas is required.
• Experience: Prior experience working with older adults, particularly in hospice or palliative care settings, is strongly preferred.
• Clinical Skills: Demonstrated strong clinical and diagnostic skills with an understanding of mental health disorders.
• Communication: Exceptional communication and interpersonal skills, demonstrating the ability to connect with clients and their families compassionately.

Preferred Experience

Compensation and Benefits
