Social Care Navigator (Bilingual Spanish)

Location: Remote - Candidates must reside in New York State or be local to the New York City metropolitan area
Schedule: Monday-Friday | 9:00 AM - 5:00 PM

We're seeking a compassionate, resourceful, and bilingual Social Care Navigator to help individuals overcome barriers related to social determinants of health (SDOH). In this fully remote role, you'll serve as a trusted advocate, connecting participants with essential community resources while providing ongoing support to improve their overall well-being.

What You'll Do
  • Manage an assigned caseload of participants, ensuring consistent outreach and follow-up.
  • Conduct comprehensive assessments by phone, video, or text to identify needs related to:
    • Food insecurity
    • Housing instability
    • Transportation
    • Financial hardship
    • Healthcare access
  • Research and connect participants with local community resources, government assistance programs, and community-based organizations.
  • Develop and manage individualized care plans with ongoing follow-up to monitor progress and address new challenges.
  • Document all participant interactions, goals, referrals, and outcomes accurately within care management systems.
  • Collaborate with internal care teams, clinical staff, and community partners to ensure seamless coordination of services.
  • Utilize motivational interviewing and trauma-informed communication techniques to engage participants and overcome barriers to care.
  • Serve as a compassionate advocate while helping participants navigate complex healthcare and social service systems.

Qualifications
Required
  • Must reside in New York State or be local to the New York City metropolitan area.
  • Minimum 2 years of experience in one of the following:
    • Care Navigation
    • Social Work
    • Health Coaching
    • Behavioral Health
    • Community Health
    • Case Management or a related field
  • Fluent in Spanish (required)-ability to communicate professionally with Spanish-speaking participants.
  • Experience using Motivational Interviewing or similar behavior change techniques.
  • Experience working with vulnerable populations, low-income individuals, or those managing chronic health conditions.
  • Comfortable working in a fully remote environment.
  • Strong computer skills with the ability to navigate CRM platforms, EHRs, or other documentation systems.
  • Excellent communication, organization, and time management skills.

Preferred Qualifications
  • Strong knowledge of New York City and New York State public assistance programs, including:
    • Medicaid
    • SNAP
    • NYCHA/Public Housing
    • Behavioral Health resources
    • Community-based organizations
  • Community Health Worker (CHW) certification.
  • Degree or formal training in Social Work, Public Health, or a related field.
  • Training in trauma-informed care and culturally responsive communication.
