• Conduct thorough psychosocial assessments to identify the emotional, social, and financial needs of patients undergoing detoxification.
• Collaborate with medical and mental health teams to devise comprehensive treatment plans that address both medical and psychological challenges.
• Provide both individual and group counseling sessions, focusing on topics such as coping strategies, grief, and adjustment issues related to recovery.
• Develop and implement discharge plans, coordinating with community organizations to arrange necessary post-discharge support and resources.
• Offer education to patients and their families on treatment options, coping mechanisms, and community resources available for ongoing support.
• Engage in crisis intervention, offering emotional support to patients and their families during challenging moments in the recovery process.
• Maintain precise and timely documentation of all patient interactions in accordance with hospital policies and legal guidelines.

The primary population you will be serving includes individuals grappling with substance use issues and seeking detoxification services. Many of these patients may also have co-occurring mental health disorders, necessitating a nuanced understanding of their psychological landscape. Your role will provide vital support as they navigate through their recovery, offering them guidance, resources, and a compassionate ear.

• A Master’s degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong communication and interpersonal skills, with a focus on establishing rapport with patients and families.
• Proficiency in coordinating care with interdisciplinary teams and community resources.

• A minimum of two years’ experience in a healthcare or hospital setting is highly preferred, particularly in mental health or addiction recovery environments.

• Experience utilizing electronic health record (EHR) systems to maintain accurate patient information.
