Work Environment

Key Responsibilities

• Conduct in-depth psychosocial assessments, collaborating with medical teams to develop individualized care plans addressing emotional, social, and financial challenges faced by patients and families.
• Provide both individual and group counseling sessions, focusing on adjustment issues, grief, and coping strategies for patients navigating complex emotions during end-of-life care.
• Coordinate with community organizations to facilitate access to necessary post-discharge services, ensuring a seamless continuum of care.
• Develop and execute thorough discharge plans, linking patients to vital community resources that support their emotional and practical needs.
• Engage in crisis intervention and provide immediate emotional support to patients and their families during challenging moments.
• Participate actively in hospital committees, contributing your social work expertise to enhance treatment protocols and improve patient experiences.
• Document all patient interactions meticulously, adhering to legal and organizational requirements while maintaining the highest standards of confidentiality.

Patient Population / Client Focus

Qualifications

• A Master’s degree in Social Work from an accredited program is required.

• A minimum of two years of experience in a healthcare or hospital setting, preferably in hospice or palliative care.
