You will be working in an outpatient office that emphasizes a collaborative and supportive culture. Our remote setting allows you to perform your duties both from home and within a team-oriented environment when necessary. • Conduct ongoing assessments of patients to identify their mental health needs and determine appropriate intervention strategies.
• Act as a liaison between supervisors, direct care staff, families, and external stakeholders, ensuring timely communication about individual progress and necessary adjustments to treatment plans.
• Develop and share a tentative schedule with supervisors that includes appointments, home visits, and observations to monitor patient progress effectively.
• Coordinate home visits to gather comprehensive data through in-home observations, chart reviews, and staff interviews to inform treatment planning and clinical decisions.
• Ensure the effective implementation of individualized program plans by collaborating closely with interdisciplinary team members.
• Participate actively in team meetings and relevant forums to discuss case progress, treatment strategies, and program improvements.
• Complete detailed monthly and quarterly progress notes documenting patient advancements and challenges.
• Organize and lead training programs for staff to enhance competencies related to the implementation of treatment plans and effective documentation.
• Conduct follow-up interviews and home visits to provide continued support for patients, promoting their mental health and well-being.
• Maintain organized and thorough records ensuring critical information is readily accessible to authorized individuals.
• Engage with the public and mental health community to advocate for patient care and organizational goals.
• Assist in developing procedural training materials to improve therapeutic programs and enhance the quality of care delivered.

• Demonstrable skills in family therapy, couples therapy, trauma-informed care, or substance misuse treatment are highly preferred.
• A strong understanding of legal and ethical standards governing the practice of counseling is mandatory.
• Exceptional interpersonal and communication skills are critical for establishing rapport and Trust with a diverse range of clients.
• Proven ability to collaborate effectively within a healthcare team is necessary.
• Proficiency in electronic medical record (EMR) systems and related technologies is a plus.

Experience working in an outpatient setting focusing on integrated care models will be considered a valuable asset. Specialized training in family systems, trauma interventions, and treatment modalities designed for youth populations is also desirable but not mandatory.
