Job Summary and Responsibilities

This position is remote with 50%+ travel.

Incumbents must reside in the Washington market

Region Director, Care Coordination

As the Region Director, Care Coordination, you will be a strategic leader responsible for optimizing patient-centered care across all hospitals in your assigned region. You will ensure high-quality care, operational excellence, and regulatory compliance within Care Management functions.

Key responsibilities include:

  • Strategic Leadership: Develop and implement comprehensive plans to drive patient throughput, efficient discharge planning, and readmission prevention.
  • Operational Oversight: Provide clinical direction for patient flow and ensure consistent application of systemwide standards and best practices for care coordination.
  • Regional Expertise: Leverage deep understanding of local regulations and resources, building relationships with state agencies and community partners.
  • Collaboration & Partnerships: Work closely with hospital and system leaders, as well as external stakeholders (payers, community resources), to achieve organizational objectives and reduce denials.
  • Performance Improvement: Drive continuous improvement in care management metrics, utilizing data analytics to enhance operations and patient outcomes.
  • Team Development: Foster a culture of excellence, provide staff development, and ensure appropriate staffing models.
  • Compliance & Ethics: Guarantee adherence to all regulatory requirements and uphold the organization's mission and values.

You will be expected to demonstrate exceptional executive leadership, foster interdisciplinary collaboration, and champion relationships to deliver critical results.

Job Requirements

Required Education and Experience

  • Master's degree or equivalent education and experience in nursing or healthcare/business related field.
  • Bachelors of Nursing and a current RN license
  • 10 years in acute care management including 5 years in a leadership role overseeing multiple facilities or a regional structure.
  • Accredited Case Manager Certified (ACM) or Certified Case Manager (CCM)
  • Proven success in developing and implementing large-scale care management strategies.
