• Conduct comprehensive case assessments for patients and develop individualized treatment plans that address their unique needs, preferences, and goals.

This role primarily involves working with patients who are in the advanced stages of illness. It requires a deep understanding of the unique emotional, psychological, and social challenges they face. You'll provide supportive counseling and resources to help both patients and their families navigate this difficult time, ensuring they feel respected and cared for throughout the process.

• A Master’s degree in Social Work from an accredited institution.
• Current Licensed Clinical Social Worker (LCSW) license in the state of Virginia.
• Proven expertise in conducting clinical assessments and developing effective treatment plans tailored to individual needs.
• Strong communication skills with an emphasis on empathetic listening and effective collaboration within a team-oriented setting.

• A minimum of two years of relevant experience in clinical social work, particularly in hospice or palliative care.
• Demonstrated ability to support individuals and families facing chronic illnesses or end-of-life issues.
• Familiarity with electronic health record (EHR) systems to document case notes and treatment plans effectively.
