• Conduct thorough psychosocial assessments to gauge emotional, social, and financial needs, collaborating with medical teams to deliver comprehensive patient care.
• Provide evidence-based individual and group counseling to patients, focusing on adjustment to illness, grief, and coping strategies for diverse life challenges.
• Coordinate with community organizations to arrange post-discharge services, ensuring smooth transitions and continuity of care for our patients.
• Actively offer emotional support and educational resources to both patients and their families regarding treatment options and community resources.
• Participate in hospital committees, providing your expertise on social work matters to contribute to broader organizational goals.
• Deliver crisis intervention and counseling, extending emotional support to patients and their families during difficult times.
• Develop and implement discharge plans, connecting patients with appropriate community resources to facilitate ongoing support.
• Work collaboratively with interdisciplinary teams, including doctors, nurses, and therapists, to enhance patient outcomes and satisfaction.
• Ensure compliance with hospital policies and legal requirements by maintaining accurate and prompt documentation.

Our patient population predominantly consists of individuals dealing with complex mental health issues, including severe mood disorders, anxiety, substance use disorders, and psychotic conditions. • A Master's degree in Social Work from an accredited program is required.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
