Position Overview
Iconic Care Management is seeking a compassionate, organized, and self-motivated Remote Medical Assistant to support our growing Principal Care Management (PCM) program. This position plays a vital role in helping patients with complex chronic conditions receive ongoing care coordination, education, and support between provider visits.
The ideal candidate is experienced in patient communication, care coordination, documentation, and working independently in a remote environment. This role requires excellent customer service skills, attention to detail, and the ability to build strong relationships with patients, providers, and caregivers.
This role is best suited for a self-motivated and confident Medical Assistant who thrives in an independent.
Qualifications:
  • Medical Assistant Certification
  • Minimum 5 years of medical office, care coordination, or chronic care management experience.
  • Strong computer skills and proficiency with EHR systems.
  • Excellent verbal and written communication skills.
  • Ability to work independently in a remote setting.
  • Strong organizational and time-management skills.
