• Conduct comprehensive psychosocial assessments for clients experiencing significant life transitions typically associated with hospice care.
• Facilitate individual, family, and group therapy sessions employing evidence-based practices tailored to client needs.
• Utilize therapeutic modalities such as cognitive-behavioral therapy, grief counseling, and supportive therapy to address complex emotional landscapes.
• Develop and implement personalized treatment plans, setting clear therapeutic goals and outlining specific interventions.
• Work collaboratively with interdisciplinary teams—including psychiatrists and psychologists—to ensure cohesive care and support for clients.
• Implement crisis intervention strategies, conduct risk assessments, and devise safety plans as required to protect client welfare.
• Maintain thorough and accurate documentation of client interactions and treatment progress in alignment with ethical and legal standards.
• Lead group therapy sessions and psychoeducation classes specifically focused on navigating grief and helping clients and their families cope with terminal illnesses.

This position primarily involves working with individuals facing terminal illnesses and their families. You'll address psychological distress, promote coping strategies, and offer therapeutic support during a challenging and sensitive life phase. Our clientele is diverse, encompassing various ages, backgrounds, and levels of mental health experience. Your ability to adapt therapeutic approaches to meet the unique needs of each client is crucial in fostering a positive impact on their overall well-being.

• A Master's degree in Social Work from an accredited institution is required.
• Current and active state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of direct experience in a mental health or outpatient setting, preferably with hospice or end-of-life care.
• In-depth knowledge of a variety of evidence-based therapeutic modalities applicable to grief and terminal illness contexts.
• Exceptional communication, interpersonal, and assessment skills to effectively connect with clients and their families.

• Experience in hospice care, palliative care, or working with clients experiencing grief and loss.
• Familiarity with telehealth platforms and virtual therapy methodologies.
• Prior experience collaborating with multidisciplinary teams in a healthcare setting.
