Position Overview

Pathfinder Counseling Services is seeking a dedicated and compassionate Licensed Mental Health Counselor (LMHC) to join our dynamic team on a full-time basis. This remote position offers the opportunity to change lives by delivering therapeutic services aimed at emotional health and wellness. As an LMHC, you will play a crucial role in developing treatment plans, conducting assessments, and ensuring that each individualโ€™s mental health journey is supported with respect and integrity.

This position is fully remote, utilizing a telehealth model to reach and assist our clients in Penfield, New York, and beyond. Key Responsibilities
  • Conduct thorough assessments and create detailed treatment plans that cater to the individual needs of clients.
  • Develop and implement crisis plans, risk assessments, and positive behavior support plans in collaboration with other team members.
  • Maintain meticulous records, ensuring timely completion of required documentation in accordance with regulatory standards.
  • Engage with evidence-based screening tools as guided by the clinical team, ensuring effective evaluation of client progress.
  • Actively participate in accreditation processes and maintain communication with relevant service systems and community partners.
  • Foster relationships with local, state, and federal agencies to promote the agencyโ€™s mission and explore potential funding opportunities.
  • Engage in ongoing professional development and represent the agency at conferences and community events.
  • Embrace the agency's mission and integrate its values into everyday practices to promote a culture of support and recovery.
  • Collaborate with the Clinic Director to perform additional duties as necessary to meet organizational goals.
Patient Population / Client Focus

The Licensed Mental Health Counselor will work with a diverse population, including individuals facing emotional and psychological challenges, those undergoing recovery from substance use, and clients with developmental disabilities or brain injuries. You will advocate for each client's unique needs, helping them navigate their mental health journey within a supportive and compassionate framework.

Qualifications
  • A Bachelor's or Master's degree in a behavioral health field is required for this position.
  • Possession of an LMHC license from the state board is essential.
  • Proven ability to maintain clinical records meticulously and prepare comprehensive reports.
