• Conduct thorough assessments to evaluate clients' social, emotional, and environmental challenges, including historical factors influencing their current situations.
• Develop individualized service plans that outline specific goals, interventions, and methods for monitoring progress to ensure effective treatment outcomes.
• Provide evidence-based counseling and emotional support to individuals and families facing significant life hardships, fostering a safe and trusting therapeutic environment.
• Collaborate directly with clients to craft personalized resource connections, helping them access relevant community services, such as housing support, medical care, and employment programs.
• Engage in crisis intervention as necessary, implementing risk assessment and safety planning to mitigate potential harm to clients.
• Work alongside community agencies to coordinate comprehensive services, articulating the unique needs of clients and advocating for their rights and well-being.
• Participate in community outreach initiatives that promote awareness of Evolve Mind Therapy's services and the resources available to those dealing with substance use issues.
• Involve yourself in case conferences, team meetings, and continuous staff training sessions to enhance your professional skills and knowledge.

This role focuses on individuals and families affected by substance use disorders within a detoxification context. You will work closely with clients who are navigating the complexities of addiction recovery, offering individualized support tailored to their unique life experiences and challenges. By focusing on empowerment and resilience, you will assist clients in overcoming barriers and achieving their personal goals.

• A Master's degree in Social Work from an accredited program is essential for this role.
• An active state license as a Licensed Clinical Social Worker (LCSW) is required to practice and provide therapeutic services.
• Demonstrated knowledge of trauma-informed care practices, as well as social and economic justice issues, is crucial for effectively supporting our client population.
• Excellent verbal and written communication skills, along with cultural competence, are necessary for building rapport and trust with clients from diverse backgrounds.
• Strong problem-solving abilities and adaptability in navigating complex social situations will enhance your effectiveness in this role.

• Prior experience working in a social service agency or a clinical setting that deals with substance use or mental health challenges is highly desirable.
